Job description
Overview

Job title: Triage Clinical Care Co-ordinator, within Care Connexions. Provide timely support to people experiencing health or social care crises in their own homes. Help triage processes and co-ordinate the clinical response to ensure seamless service delivery and timely access to hospital avoidance services.

Working Hours: 37.5 hours over a 7-day week, covering 8am–8pm.

Responsibilities
  • Support the triage processes and allocation and co-ordination of the clinical response for Care Connexions.
  • Provide clinical oversight and support to triage practitioners, ensuring triage is accurate, safe, and well-coordinated.
  • Support timely access to hospital avoidance services including 2-hour Urgent Community Response, Virtual Wards, Home First Service and third-sector services.
  • Work across Health and Social Care settings with understanding of hospital avoidance services to deploy the right services to meet patient needs.
