Position: Full-time -32.5 hours per week
Rate of Pay: £15.00 per hour

We are currently looking for a Client Assessor to join our growing team. This is a rewarding and dynamic role for someone who is confident, compassionate, and dedicated to making a difference in the lives of others.

About the role:

As a Care Assessor, you will be responsible for assessing the individual needs of new and existing clients and creating tailored care plans that reflect their goals, preferences, and well‑being.

You’ll work closely with clients, their families, and our Care Assistants to ensure the highest standards of care are maintained, and that services are delivered in line with Right at Home values and regulatory requirements.

Key Responsibilities:
  • Conduct initial care assessments in clients’ homes and develop detailed, person‑centred care plans.

  • Carry out risk assessments and ensure compliance with health and safety standards.

  • Regularly review and update care plans to reflect changing needs.

  • Build trusted relationships with clients and families, offering ongoing support and guidance.

  • Liaise with healthcare professionals and other external agencies as needed.

  • Participate in the on‑call rota, providing out‑of‑hours support as required (paid additionally).

What we’re looking for:
  • Experience in care coordination, assessing, or a similar role within the care sector.

  • A sound understanding of person‑centred care and care planning.

  • NVQ Level 3 in Health & Social Care Qualification (or working towards).

  • Excellent communication, interpersonal, and organisational skills.

  • Confident in using technology to manage documentation and communication.

  • A full UK driving licence and access to your own vehicle (essential).
