Responsibilities

• Use clinical skills and knowledge to complete the triage of referrals for patients on discharge and assess pathways from acute and community hospitals. This includes assessing the patient’s needs, determining priority of care, and processing and allocating referrals accordingly.
• Communicate effectively with the multidisciplinary team to ensure timely responses to referrals. Conduct telephone assessments and reviews of individuals in their own homes or care homes following discharge and plan appropriate therapy interventions to meet patient needs.
• Assess and treat patients in their own homes/care homes.
• Be familiar with clinical databases (Systmone, IRIS, Liquid Logic) or be prepared to receive training.
• Prioritise own workload, work as part of a team, and use initiative in a dynamic integrated care environment.
• Work as part of the Central Norfolk Home First Hub multidisciplinary team to facilitate early discharge and prevent unnecessary admissions, liaising with health care professionals and statutory/non-statutory agencies to provide a seamless service on the Discharge to Assess pathway and in community teams.

What you will do on the Discharge to Assess pathway

• Triaging and allocating referrals for patients on discharge to assess, assessing needs, determining priority of care, and ensuring timely responses with the integrated team.
• Assess and prescribe care packages for patients with long-term conditions, the frail and elderly, palliative care and rehabilitation needs to support quality of life and independence where possible.
• Support the integrated team to facilitate early discharge and prevent unnecessary hospital admissions.
• Assess and treat patients in their own homes/care homes and maintain liaison with all health care professionals and agencies to provide a seamless service.

Qualifications and Skills

• Excellent interpersonal and clinical skills; flexible, approachable and a team player with the ability to work across a seven-day week.
• Experience as an Occupational Therapist or Physiotherapist preferred; opportunities to maintain clinical competencies across Procured Beds and Urgent Community Response Therapy Team.
• Familiarity with relevant clinical databases (Systmone, IRIS, Liquid Logic) or willingness to receive training.
• Ability to prioritise workloads, communicate effectively, work as part of a team, and use own initiative.

Working for our organisation

• The Central Norfolk Home First Hub is an integrated multidisciplinary team of health and social care colleagues, including clinical leads, practice consultants, OT(s), Physiotherapist(s), community practitioner roles, and social care practitioners, supported by Integrated Care Coordinators and business support. It is a newly established and evolving team.
