A regional healthcare provider in Stockport is seeking candidates for part-time roles to support patient care in their homes. The positions require excellent customer service and IT skills, as well as the ability to manage clinical caseloads in a multi-disciplinary team environment. The ideal candidates will possess NVQ qualifications and the ability to adapt in a fast-paced setting while ensuring confidentiality and professional conduct. This position is for 1 part time 30 hours per week post and 1 part time 22.5 hours per week post, some weekend working is required.
This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.
UCR/VW is a multi‑disciplinary team that provides a two‑hour urgent response to patients in their own home that are experiencing a crisis. The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.
This role requires a highly organised person, with ability to prioritise their workload and work in a calm and effective manner and communicate well at all levels.
