Job Summary

This position is for 1 part time 30 hours per week post and 1 part time 22.5 hours per week post, some weekend working is required.

This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.

UCR/VW is a multi‑disciplinary team that provides a two‑hour urgent response to patients in their own home that are experiencing a crisis. The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.

This role requires a highly organised person, with ability to prioritise their workload and work in a calm and effective manner and communicate well at all levels.

Job Responsibilities
  • Ensure that the team case load is kept up to date.
  • To participate and co‑operate with the Multi Disciplinary Team to contribute to the effectiveness of the service as a whole.
  • To manage and maintain the team’s clinical caseload, including all associated administration tasks.
  • To use multiple patient information management systems (EMIS, Docobo, Patient Centre etc.) to enter such information as required on the computerised patient information system.
  • To use discretion, sensitivity and deal with information in a confidential and professional manner at all times.
  • Oversee and monitor the distribution/collection of equipment that is used to monitor patients in their own home (tech‑enabled care).
  • Maintain an accurate database of the home monitoring devices.
  • To be responsible for data collection, based on the outcomes from the clinical caseload.
  • To participate in and organise team audits when appropriate.
Person Specification
Essential Qualifications
  • Good general level of education, including English and Maths or equivalent qualifications or equivalent experience
  • NVQ Level 3 or above in a relevant subject such as Customer Care, IT, or Business Administration or equivalent experience.
Essential Knowledge
  • Understanding of confidentiality
  • Intermediate IT skills including Microsoft Outlook, Word and Excel
Desirable Knowledge
  • Understanding of current issues affecting the NHS and Social Care.
  • Knowledge of rota systems
  • Knowledge of Electronic Patient Record Systems

  • Excellent customer service skills with an excellent telephone manner alongside the ability to communicate clearly and accurately.

If successful this vacancy will require an standard DBS check at a cost of £26.90. This cost will be deducted from your first Trust salary.

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
