Complex Case Manager (Non Clinical)

Location: Brighouse, Field Based, Travel Required

Salary: Up to 35K depending on experience plus bonus and on call payment

Contract: Full-Time, Permanent

Role Overview
  • Work closely with your Registered Service Manager and Nurse Case Manager(s). Centralised support teams take a little off your plate and keep things running smoothly. HR, Payroll, Quality and Marketing to name but a few.

Responsibilities
  • Take ownership of a case load of complex clients with clinical interventions including spinal injuries, acquired brain injuries, motor neuron disease and multiple sclerosis; manage their care package holistically.
  • Co‑ordinate and lead the transition, start‑up and continuity of all complex care packages; involving client, MDT; managing the clinical training needs of the care team.
  • Maintain appropriate skill mix to ensure safe and effective clinical practice.
  • Uphold high standards in care and prioritise a person‑centred approach.
  • Prioritise individual wellbeing in our care practices.
  • Act as a responsive advocate for complaints and safeguarding, and encourage improvement.
  • Develop comprehensive care plans and risk assessments.
  • Provide support for your care packages through observations and mentoring sessions, and conduct regular reviews.
Qualifications
  • Someone passionate about putting people first.
  • Strong organisational and time‑management skills.
  • Good understanding of CQC requirements.
  • Experience managing complex care packages with clinical interventions including spinal injuries, acquired brain injuries, motor neuron disease and multiple sclerosis.
  • Experience managing a team of healthcare workers.
  • Driver’s licence with access to own car for work.
  • Core role include ownership of complex clients, coordination with MDT, maintaining skill mix, safeguarding, risk assessment, mentoring and continuous improvement.
