We are seeking a compassionate and motivated Carer Support Worker to join Compass House Medical Centres. This exciting role focuses on supporting unpaid carers within our local community, helping them access essential services and improve their health and wellbeing. You will work closely with GP practice teams, social prescribers, and local agencies to identify carers needs, provide assessments, and offer guidance using a strengths-based, trauma-informed approach. Main duties of the job

You will undertake Health and Wellbeing Checks and assessments for carers, identifying their needs, outcomes, and eligibility for additional support. The role includes providing guidance, referring to specialist services, and supporting carers to access community resources. You will also contribute to carers contingency planning, coding, and the Carers Passport initiative, maintain reporting processes, and help promote carers services within the practice. Collaboration with practice staff and external agencies is key, as is delivering Carers Awareness Training to staff and supporting local projects that enhance carers support.

Job responsibilities

Overview of the Role:Compass House Medical Centres are seeking a dedicated Carer Support Worker to provide support, guidance, and assessments for unpaid carers in our community. This role focuses on improving carers health and wellbeing, ensuring they are aware of and can access appropriate services and resources. You will work closely with GP practice teams, social prescribers, and external agencies to identify carers needs and deliver person-centered support.

Key Responsibilities:
