A healthcare organization is seeking a dedicated candidate to assist in delivering patient-centered care. The role involves working collaboratively with social workers and the multi-disciplinary team to provide support to service users. Applicants should have an NVQ Level 3 in Health or Social Care and a good understanding of working with diverse populations. The contract is permanent, full-time, with a salary ranging from £27,485 to £30,162 per annum. Flexible working options are available from day one.
You will be responsible for assisting with the delivery of patient centred care under the supervision of a social worker, providing practical support to service users in performing day to day activities, guidance and education, and in assisting in their management and care based upon individual needs under the guidance of a senior member of the Team.
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year, working shifts where appropriate and operationally required.

The successful candidate will work with carers and families of people with mental health needs under the direction of a Consultant Social Worker. You will support and enable the carers to connect with local assets within the communities to help build resilience.
