Job DescriptionJob Purpose
To ensure our Care Professionals and clients receive the appropriate support and a consistently high-quality service.
Key Responsibilities
Schedule and manage day-to-day care visits, including short-notice changes due to sickness or availability
Conduct Care Professional support visits and ongoing competencies
Create, update, and audit client care plans and support with digital care planning
Handle client enquiries, consultations, and manage the care journey
Maintain accurate records using Home Instead’s systems and People Planner
Coordinate and conduct Care Professional and client introductions
Build and maintain positive relationships with clients, families, and healthcare professionals
Mentor and support Care Professionals
Maintain regular contact with both clients and Care Professionals
Participate in the on-call rota
Carry out client visits as required
Uphold Home Instead’s policies, including Equality, Diversity, and Equal Opportunities
Essential Criteria
NVQ Level 3 in Health & Social Care or willingness to work towards it
Previous experience in the care sector delivering a wide range of personal care services
Supervisory experience working with MAR charts and medication administration
Strong understanding of relevant health and social care legislation and regulations
Excellent communication and relationship-building skills
Confident using care management technology and supporting others in its use
Strong IT skills including Microsoft Office or Google Suite, and virtual platforms
Full UK driving licence and access to a vehicle
Organised, adaptable, and committed to delivering exceptional service
Ability to maintain confidentiality and adhere to current data protection standards
