Qualifications

• Sign up new clients and liaise with Social Services and families.
• Complete client reviews and ensure service quality.
• Create and review person-centered care plans.
• Communicate client needs to care staff and adjust plans accordingly.
• Conduct spot checks and face-to-face supervisions.
