Overview

The successful candidate will play a key role in proactively identifying and working with people who engage with the practice irregularly and those who fail to engage with smear programmes, child vaccinations, etc. The post holder will develop relationships with hard-to-reach groups and understand how the practice can adapt to better understand and meet their needs. They will work alongside social prescribing link workers and health and wellbeing for life to provide an all-encompassing approach to personalised care and help people navigate the health and care system.

Responsibilities

• Proactively identify and work with people who engage with the practice irregularly and those who fail to engage with smear programmes, child vaccinations, etc.
• Work with frail/elderly individuals and those with long-term conditions who regularly do not attend appointments; help manage their needs and ensure access to appropriate information and services.
• Develop relationships with hard-to-reach groups and adapt practice approaches to better meet their needs.
• Answer queries, assist with booking and managing appointments, and ensure patients have good quality information to help them make informed choices about their care.
• Work closely with GPs and practice teams to ensure appropriate clinical support is available.
• Provide coordination and navigation for patients and carers across health and care services, working with social prescribing link workers, health and wellbeing coaches, and other primary care professionals to ensure joined-up care.
• Signpost patients to external organisations for non-health-related support, such as Citizens Advice, Wellbeing for Life, exercise referral, and community groups.
• Attend multidisciplinary team meetings within the practice to ensure appropriate follow-up for patients.
• Identify unpaid carers and help them access services to support them.

Qualifications and Skills

• Excellent interpersonal and communication skills; able to work confidently and effectively in diverse and sometimes challenging environments.
• Organised, patient, and empathetic with experience in health, social care or other support roles including direct contact with people, families, or carers.
