We’re recruiting for two exciting opportunities to join a growing Care Coordination team in Wokingham! Whether you’re an experienced coordinator or ready to take the next step, these roles offer genuine progression and the chance to make a real difference in delivering person‑centred care.

Care Coordinator (£28k – £35k)

Supporting a caseload of less complex care packages (with exposure to a few complex cases), you’ll focus on delivering exceptional customer service, accuracy, and responsiveness in a fast‑paced environment.

Senior Care Coordinator (£35k – £40k)

Perfect for an experienced coordinator ready to step up — managing complex rotas, leading on higher‑dependency packages, and developing leadership and mentoring skills.
