A health service provider in Wales is seeking a dedicated individual to support children and families in addressing mental health issues. The role involves working closely with a multidisciplinary team to provide comprehensive patient care across various settings. Candidates should have experience or skills in mental health assessment and planning, along with the ability to collaborate effectively with other professionals. A commitment to patient-centered care is essential, and while Welsh speaking is desirable, it is not mandatory.
The post holder will be working closely with colleagues in the multidisciplinary team and wider services to help children, young people, families and carers to address mental health issues and the resulting effects for them. To provide patient centred care of a complex nature in a variety of settings.
Work with the specialist nurse to provide on‑going assessment, planning, implementation and evaluation of patient care, ensuring the maintenance of clinical records. Also, to be responsible for developing plans of action, in conjunction with Specialist Nurse. Work with the Specialist Nurse in the management of a caseload, working closely with the multi‑disciplinary team, patient, family and any other agencies.
At times work within the community setting, involving close family members where required, with an aim to promote positive change within the family. Attend and contribute to core groups, review meetings, child protection meetings and any other clinical forums requiring key‑worker involvement and provide accurate reports for such meetings ensuring patient involvement where required.
This post is advertised as Welsh Desirable. This doesn’t mean essential; while the candidate doesn’t need to have skills in Welsh, we’ll consider it an advantage when short‑listing and selecting candidates. This isn’t “fluency”, just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
