A primary care network in London seeks a Primary Care Nurse to deliver high-quality, patient-centered nursing care. The role includes managing chronic diseases, conducting health checks, and collaborating with GPs to ensure effective patient care. Candidates should have NMC registration and recent clinical experience in a community or primary care setting.
Our Bank Nurses provide flexible support across the PCN on weekdays and weekends, carrying out nursing duties to ensure patients receive safe and effective care.
We are a Primary Care Network (PCN) based in Waltham Forest, operating as Forest Integrated Health PCN. Our network brings together seven GP practices, working collaboratively to provide high-quality, patient-centred care to our community. In addition to core GP services, we deliver extended hours clinics from Monday through Sunday, ensuring that patients have greater access to healthcare at times that suit their needs. By working as an integrated team, we focus on improving health outcomes, enhancing patient experience, and supporting the sustainability of primary care in Waltham Forest.
The Primary Care Nurse provides high-quality, patient-centred care within the PCN. Duties include general nursing tasks (such as wound care, immunisations, and health checks), supporting long-term condition management, delivering health promotion, and assisting with urgent care needs. The role involves working closely with GPs and the wider healthcare team to ensure safe and effective care for all patients.
