Job description

Central London Community Healthcare are curretly seeking a Bank Band 6 Specialist nurse for bladder band bowel

The post holder will be responsible for the assessment, implementation, evaluation and delivery of care needs to patients with continence in their own homes, care home and clinic settings

Main duties of the job
  • Autonomously assess, plan, implement and evaluate patient care
  • Provide skilled continence care to patients in a variety of community and Clinical settings
  • Possess a good working knowledge of catheter related care.
  • Act upon & monitor, record, and report symptoms and changes in patients' conditions.
  • To effect change in practice to the care of patients with continence needs, using research and evidence.
  • Demonstrate an inquiring approach towards continence care.
  • Provide professional advice to multi-disciplinary, multi-professional teams and the general public on continence issues
  • To provide continence care to a range of patients with identified continence needs.
  • Contribute to health promotion programmes to address and reduce known risk factors that affect people's health.
  • Promote healthy behaviours in patients, recognizing and supporting their readiness to change.
  • Ensure documentation is accurate and up to date and written in line with the Trust policy and the Nursing and Midwifery Council's guidelines.
  • Work with patients, carers and families to enable them to develop self-care skills with respect to continence management.
  • Ensure patient's views are taken into account in the decision-making process regarding their care.
  • Educate patients, relatives and carers where appropriate
