Ealing Short Term Rehabilitation - Band 6 Community Physiotherapist

Are you an innovative, organised and motivated physiotherapist with a strong interest in community rehabilitation? Would you like to work in a friendly and supportive environment with a focus on further learning and developing clinical skills? We have exciting opportunities for a Fixed term Band 6 physiotherapist to join our specialist multidisciplinary community rehabilitation service.

Please note that our service has 2 bases and interviews will be held in person at our office in Level 10 South, Ealing Hospital, London or EDTC, Southall UB1 2SH.

The Ealing Short Term Rehabilitation (STR) team provides therapy‑led rehabilitation to help patients improve their abilities and achieve their rehabilitation goals, through therapy led care. We focus on achieving goals and enabling people to regain independence and social integration effectively and efficiently after discharge from acute hospitals and community services in the borough. Therapists and rehabilitation assistants offer support for a period of up to six weeks, usually less if goals are achieved or care can be delivered in a different way. Our team includes Physiotherapists, Occupational Therapists, Rehabilitation Assistants. The team provide a service in a variety of community locations but mostly in the patients’ own home.

The post holder will contribute to the Physiotherapy provision within the STR team and will deliver skilled physiotherapy care to patients in their own home, based on a thorough holistic assessment of their clinical and personal needs and the agreement of an individualised rehabilitation care plan to the standards set by CSP and HCPC.

The post holder will participate in service‑wide seven day working on a rota basis.

We are a friendly team, based across Ealing borough, we support staff to be autonomous and will support you in developing your clinical, supervisory and management skills.

Responsibilities
  • To undertake physiotherapy assessment of patients, with diverse needs through subjective questioning and objective assessment.
  • To prioritise factors involved in the patient’s presenting condition through clinical reasoning based on knowledge of current professional opinion, prognosis and other physical, social emotional and mental health issues.
  • To set goals for treatment with patients and/or their carer and ensure that care is patient centred and agreed by the patient and/or their carer.
  • To set and maintain a high standard of clinical practice which is evidence based, using standardised assessment and outcome measures where appropriate.
  • To independently screen and assess multidisciplinary referrals from a variety of sources. This screening will establish priority, determine needs and initiate a plan of intervention.
  • To manage a caseload of patients with a range of needs, utilising supervision and guidance as necessary from senior staff. To successfully monitor, evaluate and review physiotherapy treatment/interventions within a multi‑disciplinary framework and to modify as necessary taking a key worker role as indicated.
  • To ensure patient and service records are recorded and maintained on IT databases and that patient documentation is according to trust and local policies.
  • To prescribe, delegate and monitor the work of rehabilitation assistants and counter sign documentation.
  • To actively be involved in service development, with support and guidance from senior therapy staff.
  • To participate in departmental continuous professional development, education and training, audit and research projects.
  • To maintain and update a Continuing Professional Development Portfolio (CPD).
