A health service provider is looking for a dedicated individual to join their virtual ward team, providing care and remote monitoring for patients with acute respiratory illness, frailty, or heart failure. Candidates should have a relevant medical qualification and experience in providing specialist care. This position offers flexible working patterns within a supportive team environment.
The Virtual Ward has been commissioned to provide a safe alternative to manage and monitor a specific group of patients with acute respiratory illness, Frailty or Heart Failure as an alternative to hospital admission and/or to facilitate earlier discharge from hospital. The patients' care will be delivered in their own home by utilising cutting edge patient friendly technology to provide remote monitoring, along with additional tailored in person or telephone support.
