Job description
Overview

The post holder will work in the Islington Rapid Response and Virtual Ward (RRVW) team, part of the Islington Urgent Care Response service that supports hospital discharges and admission avoidance. RRVW is co-located with therapy and adult social services and aims to respond to patient referrals in the community within 2 hours to prevent or reduce hospital admissions. The post holder will primarily work in Islington but may be required to see patients in other North Central London boroughs and may support UCLH@home and NMUH VW.

Responsibilities
  • Proactively manage a caseload of patients requiring complex case management to prevent hospital admissions and facilitate early discharge to improve health outcomes.
  • Identify patients for case management via screening in the Ambulatory Emergency Care unit, Accident and Emergency departments, UCLH wards, and via community referrals (e.g., GP, care homes, community nurses, LAS).
  • Provide specialist, high-quality, expert nursing care and support for patients suitable for Islington Urgent Response and Recovery team care in liaison with multidisciplinary teams (MDT) through assessment and care planning.
  • Initiate agreed range of diagnostic tests and refer to other disciplines/professions when required.
  • Support patients to self-manage, monitor their condition and make informed choices.
  • Act as a named nurse for designated patients within the service.
  • Ensure nursing documentation is maintained to high standards and participate in ongoing development of the Trust's planned Electronic Patient Record; promote evidenced-based practice.
  • Participate in clinical supervision to agreed standards; ensure high standards of care are given to all patients.
  • Develop patient condition-specific management plans with medical staff to improve clinical management and ensure pathways and procedures relating to the Islington Urgent Response and Recovery team are communicated to other clinicians and team members.
  • Disseminate policy and procedural changes in a timely manner and work within the Islington Urgent Response and Recovery team.
  • Promote the service and support clinical staff to facilitate safe and appropriate transfer of patients from ED, wards and the community.
  • Work with and be responsible for the case management of patients identified for the Islington Urgent Response and Recovery team.
  • Attend internal and external meetings as a representative of the department/Trust in relation to the Islington Urgent Response and Recovery team nursing role and responsibilities.

  • Familiarise yourself with and comply with the London Child Protection Procedures and related safeguarding protocols.
  • Familiarise and comply with the London Multi-Agency Safeguarding Vulnerable Adults Pan-London Procedures.
  • Be familiar with local information-sharing protocols and reporting lines.
  • Participate in required training and supervision and maintain professional boundaries and codes of conduct.
