Main duties of the job

As a Band 5 Community Nurse, you will be an essential part of our24hour district nursing service, delivering high-quality, holistic care to patients in their own homes. Working under the direction of the District Nursing Team Manager/Leader, you will:

• Carry delegated responsibility for theassessment, planning, implementation and evaluationof evidencebased nursing care.
• Provide skilled clinical care that supports patients to maintain independence and quality of life at home.
• Supervise and support team membersin the absence of the District Nursing Team Leader when required.
• Work collaboratively with multidisciplinary colleagues to ensure safe, coordinated and personcentred care.
