Overview

We are seeking a skilled and autonomous Advanced Nurse Practitioner (ANP) to join our dynamic Clinical Assessment Service (CAS) based in Stockley Park, Uxbridge. Our CAS team delivers high-quality, evidence-based telephone assessments and clinical advice to patients across the region. Working closely with GPs, specialist clinicians, and a multidisciplinary team, you will be at the forefront of urgent primary care delivery. The CAS model is designed to improve patient access to the right care at the right time. As an ANP, you will work within agreed clinical protocols to assess patient needs, provide timely clinical advice, and make appropriate onward referrals. You will be supported by a collaborative and professional team, while being trusted to manage your own clinical caseload and contribute to continuous service development. This is a flexible role that can be undertaken either remotely from home or in our modern offices in Stockley Park, though office-based working is preferred to enhance team collaboration and support. Previous CAS or Out-of-Hours (OOH) telephone assessment experience is essential for this role.

Main duties of the job
  • Conducting telephone assessments of patients' presenting complaints and providing timely, evidence-based advice or referrals using established clinical protocols.
  • Offering self-care guidance or directing patients to appropriate services including their own GP, urgent care, or out-of-hours services.
  • Collaborating with GPs and other healthcare professionals to ensure optimal clinical outcomes and patient pathways.
  • Maintaining accurate, contemporaneous clinical records using the system in place.
  • Educating patients on appropriate use of primary, urgent, and emergency care services.
  • Supporting the professional development of other team members through teaching and assessment.
  • Participating in audits and contributing to the continuous improvement of the service in line with clinical governance frameworks.
  • Promoting effective communication and teamworking to ensure seamless patient care across settings.
  • Taking a person-centred approach in all aspects of service delivery, demonstrating understanding and experience of integrated care pathways.
About us
  • Registered Nurse (1st level) with current registration with the NMC.
  • Post-registration qualification in autonomous practice or equivalent, and an Independent Nurse Prescriber.
  • Substantial post-registration experience, including work in A&E, Primary Care, Minor Injuries/Illness Units, or acute specialties at a senior level.
  • Demonstrated ability to work autonomously, with strong clinical reasoning and decision-making skills.
  • Proficient in physical assessment, diagnosis, and treatment planning supported by a clinical portfolio.
  • Experience in teaching, mentoring, and working within a multi-disciplinary team.
  • Excellent verbal and written communication skills with basic IT proficiency.
  • Commitment to Continuous Professional Development and maintaining clinical competencies.
  • A clear understanding of service delivery models and a passion for improving patient access and outcomes in urgent care.
