Role: Advanced Nurse Practitioner / Advanced Paramedic / Advanced Clinical Practitioner
Location: Waltham Forest Adults Community Health Services - Unplanned Care

Team: Rapid Response Team – a progressive multidisciplinary team providing urgent MDT response to prevent avoidable hospital admissions, operating across 7 days a week, 24 hours a day, to support and treat suitable patients in the community with escalated health and social care needs.

Responsibilities
- Work collaboratively with health and social care professionals across the borough and Integrated Care System.
- Lead clinical decision making and undertake enhanced holistic assessment of patient/family needs in partnership with the patient and their family/carer.
- Triage referrals from other agencies or disciplines.
- Coordinate intensive and complex nursing care with other multidisciplinary agencies.
- Be proficient in advanced clinical skills with the ability to assess physical and psychosocial needs and develop a comprehensive care plan with the patient, carers and the wider MDT as needed.
- Work collaboratively and carry out joint visits with MDT colleagues to promote holistic person-centred assessment and approach to care.
- Participate in managerial and clinical supervision; provide line management supervision to Rapid Response team members.
- Contribute to and carry out audits within the service.
- Conduct and contribute to Datix investigations/Root Cause Analysis/Serious Incidents and support lessons learned.
- Support and mentor pre-registration learners from a variety of disciplines.
- Be a driver with access to a car.
Qualifications & Experience
- Registered Nurse (Part 1 of NMC register) or Health & Care Professions Council e.g. Paramedic
- Non-Medical Prescriber
- Postgraduate level study or demonstrated postgraduate level working via CPD
- Certificate/Diploma in Management Studies Specialist Community Practice Teacher or equivalent
- Significant post-registration experience in relevant areas at Band 6 or above; experience in community or urgent care response teams
- Experience in management, quality improvement processes and clinical audits
Knowledge & Skills
- Ability to organise and prioritise clinical caseloads within a multidisciplinary team
- Up-to-date knowledge in Urgent Care / 0-2 hour response
- Clear vision of the role and service; proficient written and verbal communication
- Understanding of health needs of diverse urban populations with deprivation
- Proficient IT skills (e.g., Word, data entry) or equivalent

Remuneration & Contract
£47,810 to £54,710 per year pro rata (plus High Cost Area Supplement).
Contract: Permanent
Working pattern: 7 days a week, 24 hours
