We are looking for a highly motivated, experienced Advanced Clinical Practitioner to join our well-established clinical team for up to 37.5 hours per week.

Responsibilities
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  • Carry out face to face and telephone, and triage patients appropriately.
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
  • Work with patients in order to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self‑care.
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NHS Standards for Mental Health (NSF).
  • Promote safe clinical decision making and a high level of independent care including assessment, diagnostic and prescribing skills.
  • Provide independent prescribing within the scope of professional practice.
  • Collaborate with GP team managing same‑day appointment requests and home visits.
  • Ensure all appointments are 15 minutes and patients are appropriately triaged to green or amber cases.
  • Maintain accurate, contemporaneous and complete patient records in line with legislation, policies and procedures.
  • Effective time‑management and prioritisation of workload to maintain quality of care.
  • Participate in quality governance systems, audit cycles and continuous improvement initiatives.
  • Support staff development and act as a positive role model within the practice.
  • Contribute to development and implementation of local guidelines, protocols and standards.
  • Maintain knowledge of domestic violence, vulnerable adults, substance abuse and addictive behaviour guidance.
Qualifications
  • Professional registration with NMC or HCPC.
  • MSc in Advanced Clinical Practice or equivalent.
Desirable qualifications
  • Mentor or teaching qualification.
  • Qualified Triage Nurse.
  • Minor Illness Qualification.
Experience
  • Management of minor illness, minor ailments, and injuries.
  • Experience of working in primary care.
Desirable experience
  • Clinical leadership.
  • Clinical experience at a senior level.
Knowledge and skills
  • Advanced clinical practice skills including management of patients with long‑term conditions.
  • Management of patients with complex needs and chronic diseases.
  • Practical understanding of primary care and community services.
  • Implementation of best practice.
  • EMIS experience (desirable).
