A local government authority in Gloucester is seeking a full-time Adult Social Care Practitioner. We have a full time (37 hours) Adult Social Care Practitioner position available in our Hospital Discharge and Assessment Team. The nature of this post requires travelling around the County. Our supportive and engaged team structure includes an Integrated Social Care Manager, two Deputy Social Care Managers, Social Work Leads, Social Care Leads, Care Navigator Coordinators, a Senior Social Worker, Social Workers, Adult Social Care Practitioners, Care Navigators and an administration team. The role focuses on promoting independence, enabling individuals to remain in their own homes and reduce the need for hospital admission or long-term care. The ideal candidate will have experience in a health and/or social care setting, knowledge of the Care Act, and the ability to apply it in practice.
