Rochdale Adult Care are recruiting for an experienced social worker to join the North Integrated Neighbourhood Team.

We work across friendly, dynamic teams alongside our district nurse colleagues with a focus on partnership working to achieve best practice and desired outcomes for our residents. We take pride in everything we do to ensure our services continue to be of an excellent and high quality standard.

Responsibilities

• Deliver and review innovative services for residents as part of the North Integrated Neighbourhood Team.
• Work within and contribute to a multi-agency and district nursing supported environment.
• Collaborate in partnership-based practice to achieve best possible outcomes for residents.
• Assist with ensuring services are of high quality and meet professional standards.

Qualifications

• DIPSW or equivalent qualification in Social Work
• Social Work England registration
• Enhanced and in date DBS certificate.
• Excellent communication skills and be able to make good decisions based on information available.
• Can work alone and within/contribute to a team/multi-agency set ups
• Possess knowledge of service needs.
• Competent IT skills and be able to navigate electronic file/recording systems.
• Can write clear, concise, and robust assessment/reports/files.
• Have a flexible approach to work and working hours when the need arises
