Social Services - Transitional Care Assistant

• Facilitate/arrange the client’s transportation home utilizing local transportation services and/or transport the client’s home by driving the agency vehicle when needed.
• Support clients in understanding and implementing hospital discharge instructions and create an individualized plan of care following agency assessments and client needs surveys.
• Complete/arrange errands to meet the client’s physical needs to support their recovery at home (nutrition, medication, medical supplies, equipment, etc.) as required, in accordance with the established service plan.
• Plan, arrange, procure, or perform home management duties (cooking, cleaning, laundry, etc.), in accordance with the established service plan and in advance of discharge when identified.
• Facilitate client and caregiver engagement, providing information on issues surrounding transitions from hospital to home and/or prevention of hospital admission and readmission.
• Communicate recommendations and support clients, caregivers, and those in the circle of care to address and repair safety concerns.
• Provide companionship to older adults suffering from social isolation.
• Complete risk analysis of home and evaluate client’s risk of falls using standardized risk assessment tools.
• Support clients post-discharge to remove barriers and reduce risks in accessing resources (pharmacy blister packs, assistive devices, referrals, etc.) and follow-up on discharge-related appointments.
• Provide system navigation for clients and caregivers.
• Communicate orally and document according to documentation standards electronically; record concerns, observations, and needs of clients in the appropriate database and inform the Program Coordinator or designate.

• Minimum of 1-2 years post-secondary education in social services, gerontology, therapy assistance, or a related field, with at least three years of job-related experience or an equivalent combination of education and experience.
• Previous knowledge of medical and technical terminology is an asset. Candidates without this knowledge are expected to develop it in the role.
• Fluency in English is required; French is an asset.
• Excellent computer skills, especially in Microsoft Office.
• Knowledge of CIMS and other databases is considered an asset.
• Valid driver’s license and access to a reliable vehicle at all times.

• The work is performed Monday to Friday in various settings including an office, hospital, community, agency vehicle, personal vehicle, and client homes.
• Travel between the office, hospital, and other locations.
• Overnight stays may be required for long-distance driving.
• Work hours are approximately 35 hours per week with potential irregular hours without notice.
• This role involves physical activity such as bending, stretching, repetitive motions, climbing, and assisting clients with mobility, including pushing wheelchairs.
