A community health service provider is seeking a candidate for an exciting role aimed at enhancing the eating disorder service in the Niagara Region. You'll work closely with families and service users to deliver vital support, particularly during mealtimes. The ideal candidate should possess knowledge of mental health, show strong organizational skills, and have relevant qualifications in social or health care. This post is an exciting enhancement of the existingmulti-disciplinary community eating disorder service with a specific focus onalternatives to admission. The team will work closely alongside the existingteam and the successful candidates will have the chance to be a part of thedevelopment of this innovative specialist pathway of care for service users inNorth East Lincolnshire. The team will provide a package of enhanced mealtimesupport and intensive work with young people and families, usually within theirown home with the aim being to prevent hospital admissions or shorten them ifthey are deemed necessary.
The mainelements of the clinical work will include supporting service users and theirfamilies (when appropriate) during mealtimes in a variety of settings, butmainly in the family home, using an evidence-based model and care pathway,completing physical health checks and supporting the MDT to deliver anintensive package of support to service users and their families.
Thepost holder may also support co-facilitation of support groups, training andassessments alongside senior members of staff.

Thepost-holder will work with all members of staff within Rharian FieldsSpecialist Community Eating Disorder Service.
They willwork in close liaison with health and social care professionals in statutoryand non-statutory agencies to facilitate and deliver a range of services toensure that continuing care needs of service users are met.
Promote andimplement creative ways of working with service users.
- Instigate,develop, and maintain a relationship with the service user and/or carer tofacilitate the accurate delivery of care
- Facilitate physicalhealth clinics or support groups and at times complete clinical sessions whensenior staff are not available.
Attendappropriate training as identified in supervision, e.g., reflective practice,handover, appraisal and provide feedback to the team on training.
Maintain andupdate skills/knowledge relevant to the post and ensure practice is evidencebased.
Attend andparticipate in team and clinical meetings as required.
Accruespecialist knowledge and training, gained through formal training andexperience, relating to mental and physical health and apply this knowledgewhen delivering care to service users.
Contributeto discussions in relation to decisions about the range of options fortreatment and care plans based on judgement of service users need.
Participatein the assessments and reviews of both service users and carers in line withteam operational policies.
