With the anticipation of opening a new building in March 2026, we are searching for a Social Worker to join our Shannex Transitional Health Services Limited team providing transitional care services at West Bedford Transitional Health in West Bedford, Nova Scotia.
Through an innovative partnership with Nova Scotia Health (NSH), Shannex proudly operates the province's first transitional health facility in West Bedford. This facility provides specialized care for NSH patients in the central zone who no longer require hospital care but need additional support and services before safely returning home or to their community.
The Social Worker is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.
The Social Worker focuses on relationships between people and their environments, which are integral to psychosocial, emotional, and spiritual wellbeing. Social Workers build upon the strengths of patients/families by focusing on existing coping strategies and problem-solving skills, past successes, and personal resources.
The Social Worker supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.

About the Opportunity
  • Completes an initial and on-going bio-psychosocial assessment to identify strengths and challenges that will influence optimal health outcomes and determine psycho-social risks for recurrent illness, disability, and health distress.
  • Develops a restorative action plan with individualized interventions to address the following, but not limited to: funding needs, housing needs, resource needs, trauma, suicide and crisis, psycho-educational needs, addictions, grief and loss, stress management, life altering transitions, mental health issues.
  • Develops and facilitates psycho-educational activities/groups for patients, and families to increase self-care and self-management.
  • Provides orientation to students, develops, and delivers presentations on pertinent topics and provides formal and informal peer mentorship.
  • Develop collaborative partnerships with community and government partners to identify and address systemic barriers enabling the patients, families, and stakeholders they work with to enhance their health and wellness.
About You
  • Bachelor of Social Work
  • Current registration with the Provincial Association of Social Workers
  • Minimum of two-years clinical experience
  • Ability to provide a clear criminal record and vulnerable sector check upon hire
  • Communication requires motivating, persuading and negotiating, involves sensitive, complex or controversial issues using considerable probing, investigation and explanation to gain acceptance of ideas.
Asset
  • Previous experience supporting admission and discharge of vulnerable populations with complex social situations; advising on care plans and implementing resources, giving the patient /family every opportunity to return to home/community.
  • Masters in Social Work
