As the Registered Nurse Case Manager, you will partner with our Director of Patient Care Services and Interdisciplinary Teams to further that mission. These passionate teams will depend on you to care for and support our patients during their hospice journey.

On An Average Day You Will

  • Spend quality one-on-one time with patients and their families to support everyday needs
  • Care for patients by providing initial and ongoing assessments of patient and family needs
  • Create and initiate care plans through assessing the history of current and previous illnesses
  • Record important health information including temperature, pulse, respiration, and blood pressure
  • Administer medications and treatments as prescribed by the physician
  • Provide health care instructions to the patient as appropriate
  • Record symptom management changes and outcomes
  • Complete, maintain, and submit accurate and relevant clinical notes regarding the patient’s condition and care given
  • And additional duties as assigned

Qualifications

  • An enthusiasm for providing the best possible care to patients and their families
  • A Registered Nurse license in good standing
  • 1 year of experience in hospice or home health preferred
  • A valid driver’s license
  • Current CPR certification
  • Excellent observation, verbal, and written communication skills
  • And additional related duties as needed
