As a member of an inter-professional team reporting to the Care Manager, the role of the Registered Practical Nurse (RPN) is to ensure the provision of excellent care and support to the patients of hospice house, their families and caregivers, as outlined by hospice house policies and procedures and the Standards of Nursing Practice of the College of Nurses of Ontario. The RPN will liaise with the interdisciplinary team to support patients and families and will provide supervision to the PSW, and support to volunteers if required. The RPN supports the efficient operation of the house through continuous quality improvement and fulfills their duties in a manner that is consistent with the mission, vision and values of Providence Care Centre.
Skills & Qualifications
- Registered Practical Nurse diploma
- LEAP Core Certification (Learning Essential Approaches in Palliative Care), or willingness to pursue.
- Unrestricted registration with the College of Nurses of Ontario
- Current CPR and First Aid certification
- At least 2 years of demonstrated experience providing hospice palliative care in community, hospice, hospital or long-term care setting.
- Prior experience in pain or symptom assessment and management.
- Prior experience working with an interdisciplinary team including volunteers.
Position Responsibilities
- Assume responsibility for all aspects of patient care in accordance with the College of Nurses of Ontario Standards & Providence Care policy and procedures
- Contributes to the assessment, development, evaluation and on-going review of the patient’s care plan and responds to patient health status changes using the hospice philosophy.
- Invites, facilitates and respects the involvement of the patient, their family/caregiver in discussions regarding the plan of care and shares knowledge as appropriate to assist them in determining health care needs and making informed decisions.
- Identifies opportunities for patient’s participation in decisions that affect the patient’s rights and lifestyle and assists staff in encouraging the patient’s participation in the decision-making process.
- Recognizes the importance of maintaining a home-like environment.
- Follows the care plan, collaborates with the interdisciplinary care team as appropriate, and reports any requirements to the Care Manager
