What you’ll be doing:

  • In collaboration with the interdisciplinary team, deliver comprehensive clinical and mental health assessments, care planning, quality patient care and education as well as discharge planning
  • Contribute to program development to enhance and support patient care
  • Provide care and comfort measures; monitor and assist with diagnostic and therapeutic efforts, while continually monitoring patient’s response to treatment
  • Document and communicate patient’s beliefs, values and wishes, keeping in mind respect for others and legal, ethical and professional boundaries
  • Maximize the patient’s participation and control in their own treatment, providing information and support for decision-making
  • Maintain awareness and demonstrate sensitivity to culture, situation and family dynamics
  • Administer psychiatric and non-psychiatric medications and treatments accurately and safely according to policy and procedure, monitoring the patient for difficulties, therapeutic responses, untoward effects and reactions and reporting effects to a physician or clinical manager
  • Ensure post-discharge follow-up is in place, accessible and has been understood by the patient
  • Complete documentation according to professional standards, the principles of clear communication and Homewood policy and procedures
  • Participate in Continuous Quality Improvement (CQI) efforts to improve safety, knowledge, skills, competency, communication and problem solving
  • Participate in committees and task forces within the health centre

What we’re looking for:

  • Bachelor of Nursing degree
  • Recently registered with the College of Nurses of Ontario (within the last 12 months) or about to be registered as an RN
  • Current BCLS certification
  • Excellent verbal and written communication skills and active listener
  • Outstanding interpersonal and relationship building skills and the ability to work effectively as part of an interdisciplinary team
  • Strong physical assessment skills and demonstrated commitment to patient-centered care
  • Critical thinking and problem-solving skills
  • Proficiency with Microsoft Word, Excel and PowerPoint
  • Understanding of the healthcare system and the services available for the treatment of mental health and addiction
  • Awareness and acknowledgement of the value of using measurement-based care when supporting patients/clients to help guide clinical decision making; experience an asset
