Salary Range : $41.76-49.99 per hour

Employment Term : Contract November 2025- March 31st 2026 (possibility of renewal until Nov 2026) Full Time (35 hours per week) (Monday-Friday business hours potential for evenings)

General Description :

The Wijewinen Health Centre is a new project of the MNFC and we are looking for one (1) full time time Registered Nurse (RN) to help cover a leave. The Wijewinen Health Centre is a Primary care facility located in central Halifax / Kjipuktuk that offers healthcare services to the urban Indigenous community.

At Wijewinen Health Centre the role of the Registered Nurse (RN) in family practice partners with and complements that of the Primary Care Providers (PCPs). The RN follows the patient through all stages of growth and development through degrees of wellness and illness from one setting to another collaborating with other RNs physicians and expert providers in various settings. The RN provides holistic assessments and creates linkages with appropriate community resources.

The RN works in partnership with the PCPs and other members of the health-care team to provide care to the entire patient population. The RN focuses on providing assessment screening healthy lifestyle support education acute episode support and chronic mental health management with a goal of improving health outcomes and facilitating access to services.

Key Responsibilities :

• Complete initial and ongoing client assessments (clinical and diagnostic reasoning) care planning case management evaluation of client competence in self-care and other related outcomes coupled with the provision of nursing care
• Communicate effectively with patients families public community partners primary care team members and other professionals and agencies using verbal written and electronic communication means
• Demonstrated critical thinking skills within scope of practice including clinical judgment and creative problem-solving skills and the ability to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively
• Performs a focused health assessment that includes health history and focused physical evaluation; considers the patients psychosocial emotional ethnic cultural and spiritual dimensions of health including his or her understanding of their health / illness experience and how their daily life is affected. This assessment is to be communicated with the attending physician for the purpose of finalizing a care plan.
• Supports the development of a long-term program to provide full wrap around supports to individual who are at risk for their child being taken into foster care.
• Collaborates with client and circle of care around development and implementation of multi-disciplinary care plans.
• Provide point-of-care counseling sessions / brief therapeutic interventions as indicated.
• Administers treatments ordered by physician such as long-acting injectable antipsychotic medications sublocade injections and perform medication adherence monitoring as indicated.
• Participates in / chair collaborative practice case rounds
• Encourages maximum independence and accountability for self-care
• Discusses treatment options with patients and involves them in decision-making and self-management
• Assesses current knowledge education and literacy levels social supports learning preferences and other factors that may affect the education approach and plan.
• Acquires develops and evaluates teaching materials and tools with consideration for cultural language physical intellectual and environmental factors.
• Provides support to families and communities as required.
• Assesses the community for strengths and opportunities to enhance health and monitors trends that may have an impact on health. This may be done in conjunction with the community health nurse or other community health providers.
• Participates in culturally appropriate educational services planning program development and policy recommendation.
• Recognizes personal attitudes beliefs feelings and values about health in their interactions with patients and their families.
• Maintains and applies evidence-based knowledge to the nursing process.
• Leads or participates in research initiatives.
• Leads or participates in conference presentations.
• Demonstrates the ability to reflect on personal and team practice through a systematic evaluation of professional competencies acceptability quality efficiency and effectiveness of practice.
• Participates in the development implementation and maintenance of medical directives and policies and procedures that guide practice.
• Adhere to all Mikmaw Native Friendship Centre policies.

Qualifications :

• Baccalaureate degree in nursing or diploma entry registered nurse
• Current CPR and BLS
• Minimum of three years of experience as a registered nurse
• Current nursing registration with the Nova Scotia College of Nurses
• Valid drivers license and access to a reliable car for travel for work
• Knowledge of primary health care and the social determinants of health
• Knowledge of concepts of health promotion disease prevention behavior change counselling program planning individual and group counselling
• Knowledge and awareness related to etuaptmumk (two-eyed seeing) cultural safety cultural humility and trauma-informed care
• Nursing care knowledge related to primary care / community health / chronic disease management
• Experience working in a family practice community or primary health care setting is an asset
• Experience with phlebotomy is an asset
• Experience with diabetes education and foot care is an asset
• Mikmaw language proficiency would be an asset but is not required
• Indigenous Ancestry is an asset please self-identify
