Brooks Long Term Care is a 24-bed unit, with 15 long-term care beds and 9 transition beds, located in a rural setting. The Registered Nurse - Long Term Care works as a charge nurse within an interdisciplinary team to provide resident and family-centered care. As a member of the CoAct team, you will be responsible for assessing, designing, and implementing continuing care services for a caseload of clients. You will play a key role in resident care planning (interdisciplinary including resident and family) to help residents achieve their goals and enhance their quality of life. Using principles of patient and family-centered care, you will collaborate with the interdisciplinary team and clients to develop individualized care plans and support clients in meeting their care goals. You will demonstrate strong critical thinking skills, the ability to prioritize competing demands, and exceptional communication skills to foster strong relationships with internal and external stakeholders.

Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities, and populations, ensuring their safety and well-being. You will utilize nursing processes through critical thinking, problem solving, and decision making, as well as teach, counsel, and advocate on behalf of patients and their families. You will support an integrated, holistic approach to patient care, health promotion, and maintenance, providing safe, quality, patient- and family-centered care aligned with AHS's shared vision and values.
Please Note: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon registration as a Registered Nurse, pay will be adjusted to the applicable rate according to AHS/UNA agreements. This posting may close early if a current employee from the unit expresses interest by the deadline. The position may be filled through promotion, transfer, or vacancy procedures. This posting may also be used to fill multiple vacancies arising from the administration of modified posting provisions.
