Key Responsibilities

  • Design and implement patient-specific care plans focused on achieving optimal recovery and enhancing overall health.
  • Provide high-quality nursing care in alignment with the patient’s care plan, including comprehensive health and psychosocial assessments to support recovery.
  • Monitor patients’ conditions and coordinate health promotion, prevention, and direct care.
  • Maintain detailed and accurate clinical records, including all required OASIS documentation.
  • Communicate effectively with clinical managers and colleagues to ensure high-quality care.
  • Provide training and support to patients and their families/caregivers.
  • Function as a key point of contact between patients and community health resources.
  • Work alongside physicians and discharge planners to provide health education to patients and caregivers.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
  • Understand and adhere to the agency’s process for managing and addressing customer complaints.
  • Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
  • Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.

Qualifications

  • Must have a current RN license (state‑specific).
  • Valid BLS certification.
  • Minimum one year of experience in home health or a healthcare facility.
  • Familiarity with Medicare and Medicaid guidelines.
  • Possess a solid understanding of professional nursing principles and home health documentation.
  • Excellent communication, organizational, and time management skills.
  • Must have a valid driver’s license and access to a reliable and insured vehicle.
    Ability to visit homes that may not be wheelchair accessible.
  • Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
