Job Description
Summary

The Physical Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Responsibilities
  1. Evaluate the patient's functional status, body systems, and physical therapy needs, and consult with physicians on the care plan.
  2. Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and coordinate with the healthcare team.
  3. Observe and report patient responses and condition changes to the supervisor and/or physician.
  4. Treat patients to relieve pain, restore or develop function, and maintain maximum functioning using evidence-based techniques.
  5. Educate healthcare team members, patients, and families about the care plan, limitations, and progress.
  6. Visit patients across the service area and nearby agencies as needed.
  7. Complete progress notes after each visit and submit according to policy.
  8. Conduct patient evaluations, develop treatment plans, and report outcomes within 24 hours.
  9. Participate in staff meetings and committees as needed.
  10. Supervise home health aides and PTAs as permitted by policy and regulation.
  11. Participate in weekend rotations for service coverage.
  12. Prepare discharge summaries and condition reports at least every 60 days.
  13. Provide in-service education as requested.
  14. Adhere to therapy standards and maintain up-to-date clinical knowledge and licensure.
  15. Perform other duties as assigned.
Education and Experience
Licensure Requirements
  • Current Physical Therapy license in Florida
  • Current CPR certification
  • Valid driver’s license, vehicle insurance, and access to reliable transportation
