Position Summary:
The Ambulatory Program Nurse Practitioner is accountable to the Manager of Ambulatory Program and supports several Ambulatory Programs including but not limited to: the Nurse Practitioner clinic for unattached patients, Internal Medicine clinics, Medical Day Unit, and the Systemic Treatment Program.
The Ambulatory Nurse Practitioner possesses a master’s degree in nursing from an approved Nurse Practitioner (NP) program in accordance with the College of Nurses of Ontario. The NP demonstrates a comprehensive knowledge in Ambulatory Care sensitive conditions.
The NP works in collaboration with the interdisciplinary team and health care providers within the hospital and larger community. The NP manages, directs, and provides comprehensive care for a select patient population within the ambulatory program, ensuring clear communication is provided to primary health care providers and referring health care providers to ensure seamless transitions in care. In partnership with the interdisciplinary team and organizational leadership, the NP supports and leads quality improvements within the Ambulatory Program.
