We are seeking a skilled and compassionate Medical Social Worker to join our home care team. This role supports patients and families in navigating the emotional, social, and practical challenges associated with illness and recovery in the home setting.

Role Overview

As a Medical Social Worker, you will work as part of a multidisciplinary home care team to assess psychosocial needs, coordinate services, and advocate for patients' well‑being. Your work helps ensure individuals can receive safe, effective care at home while addressing any barriers to recovery or independence.

Responsibilities
  • Conduct in-home psychosocial assessments
  • Develop and implement individualized care plans in collaboration with the clinical team
  • Provide emotional support and crisis intervention to patients and families
  • Assist with advance care planning and end-of-life support as appropriate
  • Facilitate access to community resources such as housing, food, financial aid, and mental health services
  • Coordinate referrals for counseling, rehabilitation, or long‑term care placement
  • Participate in interdisciplinary team meetings and care planning
The Ideal Candidate
  • Master’s Degree in Social Work (MSW) from an accredited institution
  • New York State LMSW or LCSW required
  • At least 1 year of experience in a home care, hospital, or healthcare setting preferred
  • Strong knowledge of community resources and care coordination
  • Empathetic, organized, and a strong communicator
  • Comfortable working independently in client homes
  • Valid driver’s license and reliable transportation preferred

Compensation: $40.00 - $45.00 per hour
