Responsibilities
  • This exciting Case Management role functions in a unique and renowned program within our Transitional Living Center. Serves as the client/patient advocate to ensure optimal treatment outcomes.
Qualifications
  • Requires experience in a health/rehabilitation field, catastrophic case management and specifically worker’s compensation.
  • Strong interpersonal and communication skills, ability to multi-task and handle multiple priorities/deadlines, familiarity with local community resources to guide decision-making for client linkage; update knowledge of available community resources on a regular basis, ability to negotiate with payors and/or external case manager representative regarding length of stay and related issues, knowledge of and adherence to HIPPA, CARF and/or Title 22 guidelines.
  • Requires a strong desire to learn and must have an LVN or RN license issued by the State of California.
