Job Description

Reporting to the Director of Care or Director of Health & Wellness, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with provincial standards of care.

Key Responsibilities

• Work as a member of the inter-professional team applying a person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate, up-to-date, and communicated as required.
• Implement and evaluate the effectiveness of care/service plans, making adjustments as needed.
• Assist with scheduling and participate in resident care conferences.
• Facilitate resident move-in, move-out, and transfers in collaboration with the Director of Care or Director of Health & Wellness.
• Provide direct resident care, administer medication/treatments, and document care in compliance with regulatory standards and policies.
• Manage drugs and narcotics securely, administer them according to regulatory requirements and policies.
• Provide resident and family education.
• Coordinate and evaluate resident care plans with residents, families, physicians, and the interdisciplinary team.
• Coordinate nursing team assignments to ensure efficient service delivery.
• Respond to changes in resident condition or medical emergencies, providing emergency first aid.
• Teach, delegate, and supervise unregulated care providers.
• Coordinate emergency responses in the absence of senior staff.
• Address resident and family concerns, ensuring appropriate actions are taken and communicated.
• Assist in implementing infection prevention, health and safety, and quality/safety plans.
• Uphold the organization’s values, ethics, morality, and integrity as per the Code of Conduct.
• Perform other duties as assigned.

Qualifications

• Current registration as a Licensed Practical Nurse in the applicable Province/State.
• Experience in senior care and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, or long-term care are assets.
• Strong communication, critical thinking, care coordination, time management, and customer service skills.
• Sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certifications.
• Proficiency with Microsoft Office applications.
• Must provide a current (within six months) Vulnerable Sector Check including a Criminal Background Check or be willing to obtain one.
