Job Description
Reporting to the Director of Care or Director of Health & Wellness, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with provincial standards of care.
Key Responsibilities
- Work as a member of the inter-professional team applying a person-centered approach to care.
- Complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate, up-to-date, and communicated as required.
- Implement and evaluate the effectiveness of care/service plans, making adjustments as needed.
- Assist with scheduling and participate in resident care conferences.
- Facilitate resident move-in, move-out, and transfers in collaboration with the Director of Care or Director of Health & Wellness.
- Provide direct resident care, administer medication/treatments, and document care in compliance with regulatory standards and policies.
- Manage drugs and narcotics securely, administer them according to regulatory requirements and policies.
- Provide resident and family education.
- Coordinate and evaluate resident care plans with residents, families, physicians, and the interdisciplinary team.
- Coordinate nursing team assignments to ensure efficient service delivery.
- Respond to changes in resident condition or medical emergencies, providing emergency first aid.
- Teach, delegate, and supervise unregulated care providers.
- Coordinate emergency responses in the absence of senior staff.
- Address resident and family concerns, ensuring appropriate actions are taken and communicated.
- Assist in implementing infection prevention, health and safety, and quality/safety plans.
- Uphold the organization’s values, ethics, morality, and integrity as per the Code of Conduct.
- Perform other duties as assigned.
Qualifications
- Current registration as a Licensed Practical Nurse in the applicable Province/State.
- Experience in senior care and with unregulated care providers preferred.
- Education and experience in gerontology, rehabilitation, or long-term care are assets.
- Strong communication, critical thinking, care coordination, time management, and customer service skills.
- Sound clinical knowledge.
- Current Basic Cardiac Life Support and First Aid certifications.
- Proficiency with Microsoft Office applications.
- Must provide a current (within six months) Vulnerable Sector Check including a Criminal Background Check or be willing to obtain one.
