Job description

FTE: 0.6 FTE Permanent position

LOCATION: Strathmore and Chestermere Clinics

START DATE: As soon as a suitable candidate is found

Position Summary

The Licensed Practical Nurse (LPN) will work primarily on foot care but will practice as a part of a multidisciplinary health team that includes community partners to deliver wholistic patient centered care, with consideration of all social determinants of health. The PCLPN provides comprehensive team-based care to patients across the lifespan, in the patients’ medical home and at Calgary Rural PCN central clinic locations. The PCLPN may support a variety of programs and services, while exercising a high degree of critical thinking, in an autonomous but collaborative and supportive environment.

Key Responsibilities
  • Provide non-invasive foot care and educate patients on how to maintain healthy feet.
  • Autoclave and sterilize instruments when required.
  • Assess patients’ health status by taking vital signs, recording health histories, and documenting presenting symptoms.
  • Collaborate to enact patient centered care plans to support patients in their health journey.
  • Assist with procedures and primary care tasks in support of comprehensive care and ensuring patient comfort and privacy.
  • Assess screening needs, promote and complete screening as per guidelines.
  • Administer cognitive screening tests to level of competency and certification.
  • Establish patient centered SMART goals through motivational interviewing and coaching.
  • Action and coordinate tasks from other health care professionals.
  • Prepare charts and assist visiting specialists in the delivery of patient care.
  • Perform ear flush when ordered.
  • Educate patients and families about general healthy lifestyle choices and preventative health measures according to current best practice guidelines.
  • Engage in goals of care discussions with patients and families.
  • Collaborate with other health care professionals in the delivery of team-based care.
  • Maintain accurate and up to date patient records in the electronic medical record (EMR).
  • Facilitate Workshops.
  • Participate in CRPCN quality improvement projects and evaluation as required.
  • Adhere to all CRPCN polices, processes, and procedures.
  • Adhere to all LPN legal and ethical standards of nursing practice including maintaining patient confidentiality.

Educational Requirements
  • Successful completion of an accredited Practical Nurse Diploma program.
  • Current CLPNA registration in good standing.
  • CPR level C Heart and Stroke.
  • Accredited Advanced foot care certification is required.
  • Police and Vulnerable sector check in the last six months.
  • Health Change Methodology or equivalent motivational interviewing education an asset.
Experience and Skills Required
  • 5 or more years of Licensed Practical Nursing experience, preferably in a primary care setting.
  • 2 or more years of experience with foot care.
  • Experience working within an inter-professional care team model is required.
  • Experience with Electronic Medical Records (EMR).
Working Conditions
  • Clinical office environment, in member Primary Care Clinics and CRPCN centralized locations.
  • Highly autonomous and supportive work environment.
  • Travel to CRPCN work sites required.
  • Valid driver’s license & access to reliable vehicle.

The Calgary Rural PCN works together with communities, family doctors and other health team members to do whatever we can to help you be as healthy as possible. We are working together to change the delivery of health care in our communities from reactive to preventative by providing services to patients that fit their needs through supports we have in our member clinics, such as nurses, dietitians, and other health care professionals. We collaborate to provide tools and education to empower patients to work on their everyday health needs.
