Job Description

Position Purpose Summary

The LPN is responsible for the assessment, planning, implementation and evaluation of resident care. The LPN provides support and direction to Health Care Aides. The LPN is an active member of the interdisciplinary team and is accountable for providing safe and competent nursing care within the scope of LPN practice. All activities carried out by the LPN are consistent with The Salvation Army’s mission and values as well as Housing Support and Grace Manor policies, procedures and protocols.

Accountabilities

Monitoring and Assessment

  • Perform and contribute to the initial and ongoing nursing assessment of assigned Residents.
  • Collect, document and report health information from objective/subjective sources to identify health needs, monitor changes in health status, and assess residents’ ability to function independently in their home environment.
  • Document timely, accurate reports of data collection, interpretation, planning, implementation and evaluating care in accordance with Salvation Army Policies & Procedures and regulatory requirements.

Care Planning

  • Analyze information and identify priorities; initiate and/or collaborate in the development of the plan of care to meet Resident needs.
  • Provide, initiate, and reinforce teaching using developed educational materials.
  • Participate in the development of a discharge plan to meet the Resident’s needs, including identifying and coordinating resources for continuity of care.
  • Identify and initiate interventions within the LPN’s legislated scope of practice.

Care Plan Implementation

  • Implement nursing treatments and interventions using evidence-based knowledge to accomplish the plan of care.
  • Assist residents with physical requirements and activities of daily living.

Evaluation

  • Observe, report and document changes in Resident status and deviations from anticipated outcomes.
  • Evaluate and modify the plan of care as needed.
  • Make recommendations to the Health Care Team, the AHS Case Manager, Social Worker and Manager regarding changes to the treatment plan based on ongoing assessment.
  • Assess and monitor unstable/ill residents, consult with the Registered Nurse or Registered Psychiatric Nurse, or arrange transfer to acute care if necessary; communicate promptly with AHS Case Manager and/or physician when there is a change in status.

Documentation

  • Record complete and accurate data for each Resident in a retrievable form per departmental policies.

Professional Responsibilities

  • Participate in programs to develop and maintain clinical competency and professional growth.
  • Participate as a team member in quality improvement activities and nursing practice committees.
  • Demonstrate a professional image to Residents, peers, other health care providers and the public.
  • Serve as Team Leader, providing direction and supervision to HCA staff in a fair and equitable manner and adjusting assignments as necessary.
  • Provide nursing leadership to team members, residents and families, direct nursing care and crisis intervention as needed.
  • Report unskilled or unprofessional conduct and any policy violations to the Registered Nurse or Manager at the first available opportunity.

Education and Research

  • Participate in research, staff development projects, in-service education and related activities.
  • Participate in the orientation of new employees/students and precepting as assigned.

Occupational Health and Safety

  • Comply with the Occupational Health and Safety system.
  • Identify and report hazards and incidents with potential for injury to self or others.
  • Work in a safe and healthy manner per policy and procedure; participate in training and apply corrective actions as needed.

Critical Relationship Management

Internal: Works closely with residents, interdisciplinary team, volunteers, family and caregivers. External: Students from recognized programs, physicians and outside agencies.

Managerial and Financial Responsibilities

  • This position reports directly to the Residential Care Manager.
  • Maintain confidential client files and coordinate resources with other community services.
Working Conditions
  • Residential setting, fast-paced; may require lifting, twisting, bending, and extended periods of sitting or standing.
  • Occasional travel within the city and some scheduled overtime; shift work may be required.

Education and Experience Qualifications

Education, Qualifications and Certifications:

  • Graduate of an approved Licensed Practical Nurse program.
  • Successful completion of an accredited post-basic study program as required for the position.
  • Current active registration with CLPNA and maintenance of certification.
  • Current First Aid & CPR certification.
  • Criminal Record Check and Non-Violent Crisis Intervention certification.
  • Class 5 driver’s license with clean driving record.

Experience and Skills

  • Two years’ experience in a continuing care or mental health setting preferred.
  • Nursing standards/protocols relevant to the job; basic nursing care procedures; scope of practice as defined by the college.
  • Ability to balance multiple demands, work in a team, and contribute to Resident planning processes.
  • Excellent communication, compassion, attention to detail, and strong observational and critical thinking skills.
  • Professional image and ability to work with Residents and families; proficient with email, MS Word and MS Excel preferred.

Working Conditions – Additional

  • Possible exposure to angry/abusive individuals, bodily fluids, drugs and chemicals, and infectious diseases.
  • On-call 24/7 or as scheduled; may involve on-site or on-call crisis interventions and potential for physical risk.

Compensation

  • This is a casual position with no fixed scheduled hours.
  • Target hiring range: $30.62 per hour (Step 1) to $36.25 per hour (Step 4); placement based on market conditions and experience.
