Responsibilities

• Conduct brief screening and assessments for service needs.
• Provide information about child development and available services.
• Develop goal-oriented Individualized Service Plans (ISP).
• Connect families with relevant FCC services.
• Assist with appointment reminders and service coordination.
• Implement case management and clinical records.
