Community Living Toronto has been a source of support for people with an intellectual disability and their families since 1948. We offer a wide range of services including respite, person-directed planning, employment supports, residential programs, and community-based activities. We are proud to support over 4,000 individuals and their families in 80+ locations across Toronto.

Qualifications

A high-school diploma, 6 months of related work experience and knowledge of health care procedures and mechanical equipment – wheelchairs, lifts, G-tubes and other specialized equipment. Post-secondary education with focus on Social or Developmental Services may be considered as an equivalent with no related work experience required. Successful completion of internal legislated and mandated trainings is required. Valid driver’s license is an asset.

Job Summary

Contributes to the delivery of supports and programs consistent with life plans to meet the needs of individuals and enhance their capacity to function in all aspects of community living.

Responsibilities
  • Carrying out duties related to the provision of services and supports.
  • Supporting assessment of day to day needs of individuals, and in the implementation of life plans.
  • Monitoring achievement of personal goals outcomes.
  • Providing appropriate personal and physical care, including medication administration and crisis response.
  • Liaising with friends and families as required to address needs and sharing information on behalf of individuals.
  • Supporting life skills training and providing emotional support to individuals.
Hours of Work

150 hours over 4 weeks
