Overview

Family Home Visitor – Casual #25-188 (Job Opportunity). This is a casual position. Work will be offered on a regular weekly basis, but hours are not guaranteed and may vary from week to week.

Casual – Recruitment #25-188 (Non-Union). Number of Vacancies: 1. Division: Health Promotion and Vaccine Preventable Diseases. Location: Espanola Office – 800 Centre Street, Espanola; Manitoulin Island Office – 6163 Highway 542, Mindemoya. The successful candidate can work from one of these two office locations. Regular travel within the agency’s catchment area is required.

Work Location Arrangement: Partial Onsite. The position requires a combination of regular on-site, remote work that includes scheduled home visits with families. Salary: $27.05 – $30.97 per hour.

Position summary: Reporting to the Program Manager, the Family Home Visitor works with families to build on their strengths, develop their parenting skills, help them connect with resources in the community, and to promote healthy growth and development.

Program context

Within the Healthy Babies Healthy Children (HBHC) program, the Family Home Visitor works collaboratively as part of a multidisciplinary team under the guidance of an assigned Public Health Nurse. The HBHC program works with high-risk families during the prenatal period and with children from birth until their transition to school in accordance with Ministry of Children Community and Social Services. The Family Home Visitor may also work collaboratively with other community services as appropriate.

Responsibilities

• Schedules and keeps appointments for assigned family visits.
• Assists families to clarify their goals and provide support to accomplish them.
• Provides information and guidance to families to enhance their problem-solving skills to support their increased independence and resourcefulness.
• Plans and utilizes materials/resources for education and demonstration with the family.
• Provides information to families on pregnancy, positive parenting, healthy child growth and development, providing a safe, secure and nurturing environment, and problem-solving skills.
• Listens and encourages families to talk about their concerns, which may include infant/children’s health and development, their own health, employment issues, housing, education, etc. Offers encouragement, support, resources, and guidance.
• Completes accurate and ongoing documentation of family visits, telephone interactions, and meetings/case conferences with other agencies, within required program timeframes and applicable agency policies and procedures.
• Is able to work within a multi-disciplinary team environment.
• Provides regular and timely updates to the assigned Public Health Nurse regarding family’s progress, goals, and next steps within required program timeframes and applicable agency policies and procedures.
• Updates and/or consults with the Family Home Visitor Team Lead and/or manager when there are concerns regarding a family file.
• Acts with professionalism and integrity, demonstrates and adheres to policies, procedures, ethics and values of Public Health Sudbury & Districts.
• Maintains confidentiality in all issues relating to family files.
• Attends staff meetings regularly and participates in training opportunities when appropriate and available.
• Attends scheduled visits, both in person and/or using a virtual platform.
• Contributes to the development of program tools/resources as required.
• Participates in the orientation and mentorship of new Family Home Visitor staff as required.
• Tracks daily time and mileage and submit weekly to Family Home Visitor Team Lead.
• This is a casual position; the number of hours you work each week may vary. The program includes both new and returning families, and work is distributed equitably.

Requirements

• Completion of at least two years of post-secondary education in social services, child/youth/development worker program, Early Childhood Education, or a related field. Alternatively, a combination of equivalent years of relevant education and experience.
• Experience in working with and assisting high-risk families with infants and/or young children.
• Demonstrated knowledge of parenting skills required to support families in challenging life situations using a trauma informed approach.
• Familiarity with the unique needs of the community to be served, with consideration towards language, culture, ethnicity, age group and socioeconomic status. Ability to interact effectively and respectfully with diverse individuals, groups and communities.
• A good listener, who demonstrates compassion, ability to nurture, flexibility, patience and ability to tolerate rejection.
• Demonstrated ability to effectively problem-solve and help/empower others to solve their own challenges.
• Ability to interact with families in a non-judgmental, calm, respectful manner.
• Ability to recognize and effectively manage unpredictable, potentially volatile situations prioritizing the safety of all involved.
• Ability to recognize and manage personal resiliency/response related to information collected or witnessed while working in the HBHC program.
• Ability to work both as a team member and independently.
• Ability to attend scheduled visits, both in person and using a virtual platform.
• Excellent communication skills.
• Strong computer skills and an ability to learn new computer programs are required.
• Must be prompt, reliable and maintain a good attendance record.
• Advanced oral and written proficiency in French an asset.
• Satisfactory Police Vulnerable Sector Check (PVSC).
