Job Summary

The Coordinator oversees and manages the day-to-day support of the Members(persons with disabilities living independently in the community). The Coordinator is responsible for ensuring that all Members have the support they need in accordance with their approved care plan.

Reporting Relationships and Roles

The Coordinator internally communicates with all areas within the organization. The Coordinator reports to the Senior Coordinator.

Primary Duties and Responsibilities
  • Supervise the Team Leaders and the Support Workers to ensure that organization's policies and procedures are followed
  • Communicate policies and procedures to the Support Workers
  • Ensure that safe work procedures are followed
  • Fully support the goals, objectives and mission statement of SSG
  • Establish and maintain open lines of communication with Support Workers and Team Leaders
  • Interact on a regular basis with the Support Workers, communicate expectations and provide timely feedback
  • Promote effective working relationships and positive work environments while working with the Support Workers, the Members, families and other employees
  • Make sure that care plans are understood and followed by the Support Workers
  • Meet on a regular basis with the Support Workers, Members and families
  • Respond in a timely manner to all phone calls and emails
  • Ensure adherence to the standards of care as set by Department of Community Services (DCS)
  • Communicate with the DCS in a timely manner and ensure that all government and reporting requirements are met
  • Ensure communication on a regular basis with DCS Care Coordinators assigned to the Members
  • Provide a quarterly report to, and attend meetings for, the Management Advisory Committee
  • Attend and contribute in a positive manner at Coordinator meetings and special organizational events
  • Ensure and organize orientation of Support Workers
  • Schedule shifts for Support Workers
  • Organize training
  • Cover shifts if no other option available
  • Carry and use organization cell phone
  • Carry and use emergency cell phone on a rotational basis
  • Ensure that employee payroll information is accurate, approved and forwarded to Payroll in a timely manner
  • Hire Support Workers, as required
  • Conduct performance evaluation of Support Workers prior to the end of their probationary period and submit all required documents to the Human Resources department
  • Conduct annual performance reviews at the end of the fiscal year and submit all required documents to the Human Resources department
  • Terminate Support Workers, as required
  • Cooperate with the Joint Occupational Health and Safety Committee or representative
  • Build relationships with the Members, families and Support workers of SSG
  • Accurately complete Member billings on a monthly basis
  • Ensure that Member accounts and spending are supervised and accurately recorded, where appropriate
  • Interact effectively with the Members in his / her home and community
  • Support the participation in recreational / social activities with the Member based on his / her care plan
  • Ensure that support plans are in place and review on an ongoing basis, at least annually
  • Investigate work-related incidents / accidents in a timely manner
  • Obtain and maintain 7 Core Competencies
  • Perform duties assigned by the Senior Coordinator that reasonably relate to achieving your performance goals
