Clinical Supervisor, Community Support Services and Adult Day Program (ADP) – Permanent
Carefor is seeking a dedicated and skilled Registered Practical Nurse (RPN) to support our Adult Day Programs (ADP) and Community Support Services (CSS). In this role, you will provide direct client care, monitor health and well-being, administer medications, and conduct assessments using tools such as the RAI-CHA. You will develop and update individualized care plans, maintain accurate documentation, and communicate effectively with families and care teams.
As a key member of the interdisciplinary team, you will also assist with activity coordination, participate in client case conferences with Ontario Health @Home Care Coordinators, and contribute to resolving service concerns. In collaboration with the Professional Practice Team, you will support staff orientation, training, performance follow-up, and reviews. Your work will be guided by evidence-based practices and data-informed decision-making to ensure high-quality service delivery and team performance.
