Job Description
  • Reporting to: Director of Care
  • Provide clinical oversight to the care team, residents, and families in accordance with organizational policies and programs and in compliance with BCCNM standards of practice.
  • Conduct ongoing resident assessment, formulate, implement, and evaluate care plans in coordination with the Care Team.
  • Work within Point Click Care, EMAR, Pathways, and Excelleris.
  • Collaborate with a multidisciplinary care team of physicians, therapists, and other care professionals.
  • Ensure medication administration complies with policies and procedures, including dialogue with physician, pharmacist, resident, and family.
  • Perform special procedures as needed (e.g., catheterization, tubes, suctioning, suprapubic catheters).
  • Maintain medical records with clinical documentation; coordinate referrals and communication with community partners (e.g., Wound Clinician, Pharmacist, Fraser Health Authority); participate in unit and staff meetings, medication reviews, care conferences, and meetings with facility partners.
  • Oversee the admission process, liaising with transferring facilities and involving family as appropriate.
  • Perform and document InterRAI Resident Assessments as needed.
  • Promote and maintain infection control; implement infection control precautions and coordinate outbreak notifications with Public Health; collaborate with BCCDC and MHO.
  • Report risks, hazards, near misses, unsafe situations, incidents, and accidents to the Director of Care.
  • Support teamwork, understand roles of other multidisciplinary team members, and be accountable for own decisions and actions.
  • Provide input for staff performance and promote effective communication with residents, families, physicians, staff, and health-care agencies; facilitate inter-shift and inter-disciplinary collaboration.
  • Participate in continuous quality improvement, safety, and accreditation programs; attend team meetings as required.
  • Serve as a clinical resource and role model for nursing staff, providing hands-on guidance and sharing expertise.
  • Maintain current knowledge and certifications applicable to the role; adhere to all policies.
  • Assume unit nurse responsibilities when staffing is short or as needed; on-call responsibilities as required.
  • Take a leadership role in emergency situations and follow fire / disaster procedures; act as First Aid Attendant for the facility.
  • Perform other related duties as required.
Qualifications
  • RN Diploma or Baccalaureate Degree from an approved school of nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
  • Gerontology certificate preferred.
  • Current CPR certification Level C.
  • WHMIS certification.
  • Ability and willingness to undergo applicable screening and background checks satisfactory to The Salvation Army, including providing a Background Check screening.
Experience
  • Previous experience in geriatrics or long-term care is an asset.
Physical
  • Must be well groomed, maintain good physical and mental health, and be fit to perform duties that may include standing, bending, lifting, etc.

  • Must demonstrate concern and interest in providing quality care to the elderly; ability to work collaboratively with residents, families, and teams with professionalism, empathy, and patience; effective communication; and strong ethics and job performance.
