Care Partner

Are you passionate about making a real difference in the lives of seniors and community members? Incasa Aged Care & Community Services in Beaconsfield, WA, is seeking a dedicated and compassionate Care Partner to join our team.

What You’ll Do
  • Manage and coordinate individual care plans for aged and disabled participants.
  • Assess participant needs and develop tailored support strategies.
  • Collaborate with participants, their families, and service providers to ensure optimal care delivery.
  • Monitor participant progress and adjust care plans as needed.
  • Oversee participant and service budget monitoring.
  • Promote independence and community engagement for our participants.
  • Work closely with a supportive and professional team to deliver exceptional service.
What We’re Looking For
  • Minimum of 2 years’ experience (within the past 10 years) working as part of a team responsible for community-based aged care and disability services.
  • Demonstrated advanced working knowledge and experience of the Home and Community Care Programs.
  • Experience in working with participants and carers from culturally diverse backgrounds and those with dementia, frail aged, disability and mental health.
  • Proven ability in participant and service budget monitoring.
  • Compassionate, respectful, and excellent communication skills.
  • Demonstrated experience identifying and working effectively with complex consumer care needs including but not limited to social economic issues, mental health, substance and/or elderly abuse.
