The Care Manager, a dynamic leadership role, is responsible for cultivating strong relationships with referral partners and area stakeholders to stimulate growth. This role prioritizes expanding our presence in current and new markets, elevating care quality, and nurturing local team engagement. The successful candidate will have a proven track record in relationship building and scaling operations, with a strong focus on amplifying our presence and reach.

Responsibilities
  • Cultivate and sustain strong relationships with referral partners and local stakeholders
  • Oversee client assessments and reassessments to support care optimization and growth
  • Monitor and act on incidents and significant changes to ensure continuity and quality
  • Champion client satisfaction and uphold service excellence
  • Represent Kindred at regional events and in community initiatives
  • Foster a positive, inclusive team culture across staff and caregivers
  • Collaborate with centralized teams to ensure caregiver availability aligns with growth
  • Support a responsive, solutions-focused “we say yes” culture
Qualifications
  • Valid RN or LPN Lisence
  • 3+ years of leadership experience in team management.
  • Substantial Healthcare Experience
  • Demonstrated success in driving growth through market expansion & customer success initiatives.
  • Ability to analyze complex situations, make sound decisions and find creative solutions.
  • Experience leading teams through periods of growth or transition.
  • Bias towards action.
  • Candidates must reside in Southwestern New Brunswick
