As part of our Residence Care Team you will be working as part of an interdisciplinary healthcare team, volunteers and staff providing care to our residents according to their care plan, providing support and assistance to the resident and their families and / or caregivers and contributing to provide a home-like, peaceful environment.

Staff may be required to transfer to an alternate site. The rate of pay is your current Hospice Care Ottawa RPN rate based on the wage schedule in the collective agreement.

Your Qualifications : We are looking for RPN’s who possess the following :

• Work 4 shifts / month minimum
• Able to work Days, Evenings, Nights and Weekends

Key Responsibilities / Principales responsabilités

Plans, coordinates, delivers and supports a team to provide day to day end-to-life palliative care for the residents in the Hospice and their families and / or significant others.

Reporting relationship / Relevé hiérarchique

• Reports to the Team Leader(s) and / or the Director of Residence Care.
• Supervises, under the direction and supervision of the Team Leader(s), the care team, including volunteers, responsible for providing end-of-life palliative care to the residents of the Hospice, as well as support to their families and / or significant others.

Qualifications & Background / Qualifications et expérience

• Current registration with the College of Nurses of Ontario
• CNA Certification in Hospice Palliative Care or willingness to obtain
• LEAP Certification or willingness to obtain within 6 months
• A strong sense of dedication to the mission, vision and values of Hospice Care Ottawa
• Knowledge and experience in Hospice Palliative Care
• Excellent communication skills
• Leadership skills
• Understands Ethics as they relate to Hospice Palliative Care
• Ability to work as part of a team

• Provides guidance and direction to Personal Support Worker (PSW)
• Provides guidance and direction to Residence Volunteers as necessary

• Participates regularly in-Residence team meetings
• Participates on committees as required
• Participates in Hospice activities (i.e. Hike, Volunteer appreciation)

Le personnel pourrait être appelé à travailler sur un autre site. Le taux de rémunération correspond à votre taux horaire actuel à titre d’infirmier(ère) auxiliaire autorisé(e) à la Maison de soins palliatifs d’Ottawa, selon l’échelle salariale prévue dans la convention collective.

Vos qualifications : Nous recherchons des infirmiers(ères) auxiliaires autorisé(es) possédant les compétences suivantes :

Disponibilité sur appel :

• Être disponible pour un minimum de quatre quarts de travail par mois.
• Être en mesure de travailler de jour, soir, nuit et les fins de semaine.

Principales responsabilités :

Planifier, coordonner, diriger et soutenir une équipe responsable de fournir des soins palliatifs quotidiens aux résidents de la maison de soins palliatifs ainsi qu’à leurs familles et / ou leurs proches.

Relevé hiérarchique :

• Relève du ou des chefs d'équipe et / ou du directeur des soins en résidence.
• Supervise, sous la direction et la supervision du / des chefs d'équipe, l'équipe de soins, y compris les bénévoles, chargé de fournir des soins palliatifs et de fin de vie aux résidents de la maison de soins palliatifs ainsi qu’un soutien à leurs familles et / ou à leurs proches.

Qualifications et expérience :

• Inscription en règle auprès de l\'Ordre des infirmières et infirmiers de l\'Ontario
• Certification de l’AIIC en soins palliatifs ou engagement à l'obtenir
• Certification LEAP ou engagement de l'obtenir dans les six (6) mois
• Fort adhésion à la mission, à la vision et aux valeurs de la Maison de soins palliatifs d’Ottawa
• Connaissance et expérience en soins palliatifs
• Excellentes aptitudes en communication
• Compétences en leadership
• Bonne compréhension des enjeux éthique lié aux soins palliatifs en résidence
• Capacité à travailler en équipe
• Capacité à donner et à recevoir des directives
• Capacité physique à assister les clients alités ou ayant des besoins en matière de mobilité
• Certificat RCR à jour
• Bilinguisme (français et anglais)

• Fournir des conseils et directives aux préposés aux services de soutien à la personne (PSSP).
• Offrir des conseils et des directives aux bénévoles de la résidence, au besoin.
• Veiller à ce que les soins personnels prodigués aux résidents soient appropriés.
• Administrer les médicaments aux résidents conformément aux ordonnances.
• Administrer les traitements aux résidents selon les prescriptions.
• Apporter un soutien émotionnel aux résidents et à leurs familles / proches et orienter ceux-ci vers le programme de soutien familiale, si nécessaire.
• Maintenir à jours les dossiers médicaux en respectant les meilleurs pratiques en matière de documentation, tout en garantissant la confidentialité.
• Rendre compte régulièrement au chef d'équipe ou au responsable des soins.
• Appliquer les procédures appropriées en cas de décès d'un résident.
• Assumer les tâches et responsabilités confiées par les chefs d'équipe en leur absence.
• Participer aux réunions quotidiennes de l'équipe en résidence (tournées).
• Assurer la liaison avec les médecins, les pharmaciens et les résidents en médecine, en encadrant ces derniers si nécessaire.
• Encadrer les nouveaux membres du personnel.
• Participer au mentorat des étudiants, au besoin.
• Participer aux activités de formation en tant que formateur ou participant.
• Participer aux mises à jour annuelles sur la santé et la sécurité, ainsi qu’à la semaine de perfectionnement des compétences de la Maison de soins palliatifs d’Ottawa.
• Participer régulièrement aux réunions de l'équipe de la résidence.
• Participer aux comités, selon les besoins.
• Contribuer aux activités de la Maison de soins palliatifs d’Ottawa (par exemple, randonnée, remerciement des bénévoles).
