CREST Post-Diagnostic Support (CREST-PDS) is a specialized programme within the Community Outreach Team network that provides targeted support to individuals recently diagnosed with dementia and their caregivers. CREST-PDS focuses on helping clients and families navigate the post-diagnosis journey through coordinated care, resource linkages, and ongoing support.

As a Social Work Associate, you will play a vital role in coordinating client care, managing programme operations, and ensuring seamless service delivery for individuals and families affected by dementia.

RESPONSIBILITIES
Programme Operations & Coordination
  • Lead timely data collation and quarterly report submission.
  • Provide administrative and logistical support to the team in running client-related support and CREST-PDS activities.
  • Prepare necessary resource materials to be provided to clients and caregivers.
  • Coordinate programme schedules, activities, and resource allocation.
Client Support & Care Coordination
Assessment & Intervention
  • Support in initial assessment of referral cases.
  • Follow up with intervention plans as per the prescribed care plan and under guidance.
  • Provide information and emotional support to clients and their caregivers.
Case Management
  • Support care coordination and service linkages for clients receiving post-diagnostic support.
  • Monitor clients' conditions and progress, escalating to senior staff where necessary.
  • Assist in ensuring clients receive appropriate follow-up care and support services.
Data Management & Reporting
  • Oversee timely data collation and maintain accurate programme records.
  • Compile and submit quarterly reports as required.
  • Support tracking of client progress and programme outcomes.
QUALIFICATIONS
  • Minimum Higher NITEC in Community Care, Nursing, or other fields related to healthcare and community care.
  • At least 8 years of healthcare or community care experience.
  • Experience working with individuals with dementia or mental health conditions is highly valued.
  • Experience in care coordination or programme administration is preferred.
  • Strong interest in supporting individuals with dementia and their caregivers.
  • Good communication and interpersonal skills to engage with clients, caregivers, and healthcare professionals.
  • Ability to provide compassionate support to clients and families navigating dementia diagnosis.
  • Strong organizational and administrative skills with attention to detail.
  • Ability to work collaboratively with allied health professionals and follow clinical guidance.
  • Proficiency in data management and report preparation.
  • Knowledge of dialects is an advantage.
