Job description

Summary: Provide and lead social work intervention and project/event management to support the work in the Day Activity Centre/Sheltered Workshop for persons with disabilities.

Casework Management
  • Administer financial assistance and/or seek deviation for clients to allow them to participate maximally in the DAC programme as needed.
  • Conduct assessment of severity, key risks and protective factors for intervention (if necessary, Home Visit); provide counselling services to help clients and caregivers to manage issues associated with their disabilities to enable maximally independent community living.
  • This could involve maintaining case history information, psychosocial assessment, social work treatment plans, and writing social reports according to required documentation standard.
  • Provide consult and support for social workers for above. Evaluate progress and effectiveness of their interventions.
  • Work with therapists, training officers, and caregivers within a multi-disciplinary context to plan and implement client’s individualized care plan.
  • Network with other agencies to tap on community resources to provide intervention support to clients.
  • For each client, explore and document care planning intentions with caregivers.
  • Conduct closure of cases upon achieving sustainable service outcomes for clients.
Community Work
  • Identify the changing trends/needs of the landscape pertaining to the PWDs and coordinate community resources to enable maximally independent community living.
  • Network with relevant referral agencies and stakeholders serving PWDs and be part of the outreach efforts.
Group Work
  • Support social workers to plan, develop, implement and review curriculum, psycho-educational training and group work for PWDs and/or their caregivers as necessary.
  • Supervise facilitation of group work interventions and evaluate progress and effectiveness of group work interventions across the organization
Project management
  • Support social workers to plan events such as SPD open house or caregivers’ appreciation day or other projects as assigned.
  • Support and lead social workers to plan, develop, implement and review curriculum for Enabling Skills & Living Programme
Professional Development
  • Lead and support Lead Social Worker in developmental programs and supervision for personal and professional growth & development.
  • Maintain minimum requirements for renewal of membership with professional association
Admin Duties
  • Coverage as needed for programme head / supervisor with programme accountability reporting duties.
  • Other duties as required/assigned by the supervisor or Head of Division.
JOB HOLDER REQUIREMENT
  • Hold a degree in Social Work from recognized institutions
  • Have excellent communication and interpersonal skills with demonstrated ability to build relationships.
  • Preferably have minimum 7 years’ experience in social work with some experience in disability sector.
  • Have minimum of 3 years’ experience in supervision.
